【★資格條件】二、業(yè)績要求：（開標(biāo)時(shí)需進(jìn)行信息公開）1、2020年1月1日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人應(yīng)具有至少1個(gè)合同的含砂分析儀或含砂監(jiān)測儀或出砂檢測儀的供貨業(yè)績，且滿足測量原理為聲波式的技術(shù)要求。2、投標(biāo)人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。業(yè)績證明文件至少包含：1）合同復(fù)印件和2）到貨驗(yàn)收材料。投標(biāo)人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同首頁、合同簽署時(shí)間、合同簽署頁（國內(nèi)貿(mào)易合同應(yīng)有雙方蓋章，國際貿(mào)易合同應(yīng)有雙方簽字或蓋章）、貨物名稱及到貨驗(yàn)收材料（至少包含：買方接收證明或調(diào)試驗(yàn)收證明或到貨驗(yàn)收單或其他可以證明合同項(xiàng)下貨物已經(jīng)到貨驗(yàn)收的有效證明材料或″與合同或訂單金額一致的所有增值稅發(fā)票掃描件（如有清單須提供，須體現(xiàn)發(fā)票號(hào)；發(fā)票需包含合同號(hào)或物資名稱，且與合同或單個(gè)訂單一致）?！背l(fā)票外其他的到貨驗(yàn)收材料至少應(yīng)有合同甲方簽字或蓋章。）3、若合同為協(xié)議，除提供協(xié)議文本外，還應(yīng)至少提供該協(xié)議對(duì)應(yīng)的1個(gè)已完成訂單的訂單頁和相應(yīng)的到貨驗(yàn)收材料，同一個(gè)年協(xié)合同下的訂單視為1個(gè)業(yè)績。4、如業(yè)績合同買方與投標(biāo)人及所投產(chǎn)品制造商之間具有關(guān)聯(lián)關(guān)系（指與投標(biāo)人及所投產(chǎn)品制造商為同一法人或具有控股關(guān)系），則上述合同業(yè)績不接受。5、未提交業(yè)績證明文件，或所提供的業(yè)績證明文件無法認(rèn)定滿足上述業(yè)績要求的，均視為無效業(yè)績。1.
